W-9 Form
1) Once you are ready to receive your first commissions, please fill out the w9 form below to assist KETO TO GO LLC in compliance with Federal Laws. PRINT the form, fill it out, then mail it to KETO TO GO LLC, 876 Grange Road, Teaneck NJ 07666. You can also email it in to igmc@aol.com.
2) Please download the KETOFILIATE ASSOCIATE / AFFILIATE agreement which explains our policies and procedures and what you are legally allowed to say and do on the Internet as a KETOFILIATE representing KETO-TO-GO. Please sign and either mail executed agreementtoabove address or scan and email to igmc@aol.com.
